Patient and Family Advisory Council
Wentworth-Douglass Hospital believes including the voice of our patients and their loved ones is key to providing exceptional patient and family centered care. The Patient and Family Advisory Council (PFAC) was created with the goal of continually improving our care and services.
PFAC is a team comprised of patients, family members, caregivers, and others in the WDH community who advise, recommend, and participate in hospital-wide programs and initiatives. Members are encouraged to draw from their own health care experiences, the experiences of their families, and of their fellow community members.
Our mission is to:
- Help share information and implement services that affect our patients and their loved ones
- Support patients and their families in becoming informed advocates for their own care
- Initiate ideas for policies, programs, projects, and services within the patient care environment
- Provide ongoing opportunities to hear the voices, experiences, and perspectives of patients and their loved ones
If you are a member of the WDH community, we invite you to apply for consideration in joining our PFAC. To learn more about this opportunity, please complete and return the form below and you will be contacted by a member of the Patient Experience team.
